We are happy to register new NHS patients within our catchment area.
How to Register
To register with our practice, please come into reception to get a form. You may also follow the link below to download the GMS1 New Patient Registration form, which you can print off and bring into the surgery;
Patients who live in our Practice Area may register with the Practice 9-12noon and 2.00-6pm on Tuesdays, Wednesdays and Thursdays.
Prospective patients are asked to supply their NHS Number, photographic ID (eg passport, driving licence), proof of address in the last 3 months (eg utility bill, landline telephone bill (not mobile telephone bills), and complete a form with their personal details along with a simple health questionnaire. You must bring this to the reception at either surgery and you will then be given a date for a registration medical with a nurse or healthcare assistant, where you will also need to bring in a urine sample.
You will be registered with the Practice as a whole and can have appointments at either Florence Road Surgery or Bramley Road Surgery.
Please complete and print the registration form and take it to the Surgery (9-12noon and 2.00-6pm Tuesdays, Wednesdays and Thursdays) and we will make your registration appointment. Please note that you will not be formally registered as a patient of the Practice until you have had your registration medical.
You will also be required to complete a Practice Health Questionnaire which should be brought with you on your first visit to the Practice along with your printed Registration Form and ID. Please click on the appropriate link below to print the questionnaire:
As from 1st April 2016 we are required to allocate a named accountable GP to each patient.
Patients will continue to be able see the GP or nurse of their choice.

We invite all our new patients to see one of our health care assistants. This appointment enables us to collect medical information before your medical records arrive. It is also the start of screening for other medical problems. Normally, you will need to complete a registration medical before seeing one of our GPs.
University of West London Students
You will be asked to supply your Student ID Card, Passport and Student Status Letter in order to register with us.
We offer a full General Practice service for all University of West London students living within the Borough of Ealing. Consultations take place by appointment at our Bramley Road Surgery. The Practice will always try to accommodate students who need appointments outside of the dedicated session times at either Bramley Road or Florence Road.
We are also able to provide students registered at the Practice with emergency health care outside of the normal surgery hours, and we are also members of Harmoni which is a local co-operative that provides out-of-hours services when we are closed.
All consultations are absolutely confidential and information will not be given to University staff, parents or other interested parties without your express permission.
During the Holidays
Like all General Practices we are open all year. However, if you are out of Ealing during the holidays, you can obtain treatment from a local GP by registering with them as a temporary patient.
Please do not register with your GP at home during the holidays as this disrupts the service we can give you on your return. When you leave university you can register then.
