How to register with us

We aim to complete all registrations within 10 working days.

Online Registration (Over 16s only)

You can now register with Florence Road Surgery electronically.

If you are able to use your NHS login (NHS app login details), we should be able to register you without having to ask you for any additional information (as you were already authenticated by NHS Login); please note, this is the fastest and paperless way of registration.

If you are unable to use NHS Login, after we have reviewed your online application, we will ask you for proof of address within the last 3 months (i.e. recent bank statement, utilities bill, tenancy agreement or council tax billAND photo proof of identity (i.e. passport, driving licence or National ID Card.) 

Please note we do not accept driving licences as proof of address.


Summary Care Records (SCR)

when we process your online registration, we will set your SCR to “Express consent for medication, allergies, adverse reactions and additional information“.

In-Person Registration

If you are unable register online you can collect paper forms from our Reception.

We will also ask you for proof of address within the last 3 months (i.e. recent bank statement, utilities bill, tenancy agreement or council tax billAND photo proof of identity (i.e. passport, driving licence or National ID Card).

Please note we do not accept driving licences as proof of address. 

Non-urgent advice: Please Note

At the moment we ask new patients to only come to the Surgery to register on Tuesday, Wednesday or Thursday between 9am-5pm.

Please do not put the registration forms through the letter box, without speaking to Reception first. They will not be processed and will be disposed of in a confidential manner.

Children (up to and including 16 year olds)

You will be required to complete a Registration Form (GMS1), Practice Health Questionnaire (New birth, Under 5 or 5-16) and SCR Consent Form and and email these to [email protected], along with your NHS Number (if you have one). 

You will need to provide an ID (Passport or birth certificate*) and vaccination records in english (red book for children born in the UK). If any vaccinations were administered overseas, you will be contacted to book an appointment with a nurse to discuss.

Make sure you have signed the GMS1 and SCR consent forms, either on behalf of patient or the patient can sign if capable.

University of West London Students

You will be asked to supply your Student ID Card, Passport and Student Status Letter in order to register with us.

We offer a full General Practice service for all University of West London students living within the Borough of Ealing.

All consultations are absolutely confidential and information will not be given to University staff, parents or other interested parties without your express permission.

Temporary Residents

If you are not registered with a doctor but need to see one, you can receive emergency treatment for 14 days. After 14 days you will need to register as a temporary or permanent patient.

To register as a temporary resident you will need to complete a GMS3 form. Please return the completed form to the surgery.

You can register as a temporary resident with a GP in England if you are in the area for longer than 24 hours but less than three months.

During the holidays

Like all General Practices we are open all year. However, if you are out of Ealing during the holidays, you can obtain treatment from a local GP by registering with them as a temporary patient.

Please do not register with your GP at home during the holidays as this disrupts the service we can give you on your return. When you leave university you can register then.
